MCP connected, zero tools: Claude Code's 2026-07-28 switch

Since Claude Code 2.1.292, stdio MCP servers get protocol 2026-07-28 by default. If tools vanish with 'Failed to fetch tools: Request timed out', read the MCP log, set MCP_PROTOCOL_NEGOTIATION=legacy, then fix the server or wait for the client fix.
